Social Services Case Manager

Job Description

VisionCorps, a values driven organization, is looking for a full time Social Services Case Manager in our Lancaster office. This position reports to the VP of Rehab and Education.

The individual applying for this position must possess a college degree in Social Work or related field from an accredited university. A minimum of two years’ experience in Human Services field and experience working with the elderly and/or disabled community is required. Must be someone who is innovative, responsible and trustworthy. Familiarity with low-vision or non-visual reading devices preferred. Must be proficient in the use of Microsoft Office Suite.

This position is responsible for meeting with new referrals, providing information on resources, services, equipment and training opportunities, and completing the referral process obtaining all required information. Educate the public on blindness and vision loss and aspects of VisionCorps services and build and maintain collaborative relationships with local and regional organizations. Entering information related to all client contacts into the Customer Relationship Management system and maintain accurate accounting for at least 70% of hours worked each month. The person holding this position will also oversee the operations of the low vision store.


  • Ability to use hands and fingers in the use of computer keyboard, completing forms, use of office equipment.
  • Ability to sit for periods of the time.
  • Ability to travel independently and safely.
  • Must have use of personal vehicle for travel (if necessary).
  • Must have or pass Criminal Record Check, Child Abuse Clearance and Motor Vehicle check from the state of PA.
  • Provide services in client’s natural environment, home, retirement community,
  • assisted living or skilled care, etc.
  • Ability to lift/carry up to 10 lbs. when required.
